“Case Management and Preventable Hospital Readmissions”

The Patient Protection and Affordable Care Act (“Affordable Care Act”) contains a section (Section 3025, and an added 1886(q) to the Social Security Act) which established the Hospital Readmissions Reduction Program. It requires the Centers for Medicare and Medicaid Services (CMS) to reduce payments to IPPS hospitals with excess readmissions. The rule became effective for patient discharges beginning on October 1, 2012. More recently, CMS posted its Fiscal year 2015 IPPS/LTCH PPS final rule which incorporates some refinements to the original readmissions measures.
